Description
C-Care Uganda is hiring a Case Manager that coordinates patient care across the continuum to ensure high-quality, cost-effective, and patient-centered services. The role focuses on assessment, care planning, resource coordination, discharge planning, and follow-up to achieve optimal clinical and psychosocial outcomes.
Key Duties and Responsibilities:
Patient assessment & care planning
- Conduct comprehensive assessments of patients’ medical, psychosocial, and functional needs.
- Develop individualized care plans in collaboration with medical specialists, patients, and families.
- Identify high-risk patients and implement appropriate interventions.
Care coordination and continuity
- Coordinate care among physicians, nurses and external providers.
- Facilitate referrals to specialists, diagnostic services, and community resources.
- Monitor patient progress and adjust care plans as needed.
Utilization and resource management
- Ensure appropriate use of healthcare resources and services.
- Support admission reviews, length-of-stay management, and discharge planning.
- Coordinate post-discharge care, including home care, rehabilitation, or follow-up appointments.
Patient advocacy and education.
- Educate patients and families on diagnoses, treatment plans, and self-care.
- Advocate for patient needs and ensure culturally sensitive care.
Qualifications, Skills and Experience:
- Degree or Diploma in Nursing, clinical medicine, or a related healthcare field.
- Registration with the relevant professional body.
- Previous experience in case management, care coordination, or clinical practice preferred.
- Experience in a hospital or managed care environment is an advantage.
