JOB DETAILS:

Job Title (2) REF: TS/HR/PMGT/ 36: Counsellor/ Community Linkages Assistant (1)
Classification: Contract, Full-time
Reporting to: Psychosocial and Community Linkages Officer
Salary: Commensurate with training and experience
Duty Station: TASO Masindi Center of Excellence
Directly Supervises: None

Functionally Supports: Expert Clients
Main Purpose of the Job: To provide services geared towards improving the welfare of TASO Clients and their house hold members.

Key Outputs

• Participation in developing departmental work plans and budgets done
• Clients' welfare provided and ensured
• HIV/AIDS education and sensitization organized
• Clients' views presented to TASO management
• Technical guidance to support clients and expert clients provided Clients mobilized, linked and retained in care
• Programmatic reports Compiled and submitted

Key Duties and Responsibilities
• Participate in the compilation of departmental annual work plans and budgets for client's activities
• Participate in writing fundable proposals at TASO units to secure local funding and link clients to support groups
• Share/disseminate information to clients regarding their rights and obligations
• Identify, document clients needs and share with management
• Orient clients about TASO center services & procedures i.e. MIPA-GIPA guidelines and Clients' Code of Conduct
• Attend to weak and support clients who are stranded i.e. do not know where to go or have no support like transport back home
• Follow up clients admitted in the wards and homes and report respective Counsellors
• Conduct health talks and spearhead clients' involvement in HIV/AIDS education and sensitization
• Conduct Training Needs Assessments and organize skills building trainings for Expert Clients Offer counseling to clients with different concerns and visit CDDPS to offer peer support counselling to clients/refills
• Support in establishment and maintenance of the demonstration gardens at the center
• Mobilize and organize targeted HTS in the community to identify HIV positives
• Link all the HIV positive clients identified during HTS and make follow ups
• Identify clients who miss appointments and fast track those who are lost to follow to ensure that they are traced and restarted on treatment
• Monitor clients' flow and waitingwhile at the center and update the appointment register, missappointment register
• Reach out to partners of clients that recently test HIV positive and invite them to the clinic for HIV counselling and testing services.
• Track client referrals and enrol those referred into the TASO System
• Work with the Psychosocial and medical team to identify, follow-up and support clients that miss appointments and those that disengage from care.
• In collaboration with Community structures, conduct community follow up of clients that cannot be reached on phone
• Prepare and submit reports on client activities as required
• Review outputs presented by facility Expert Clients and ensure that proper documentation of work is done

PERSON SPECIFICATIONS:
Counsellor/Community Linkages Assistant
Minimum Academic Qualifications: Degree in SWASA, Education, Social Sciences, Development Studies, Counselling and Guidance, Public Health, Psychiatric Nursing, Clinical Psychiatry OR Community Psychology.

Additional Training Necessary: Certificate in HIV/AIDS Counselling and Certificate in Child and Adolescent Counselling.

Minimum Experience and Exposure:
• At least 2 years' experience in HIV/AIDS support work.
• Ability to communicate in the local dialect and knowledge of the area project scope is a MUST.
• He/she MUST be a TASO Registered Client and, evidence regarding the same shall be required for verification.
• Motorcycle riding is a Must. The applicant must attach a copy of a valid riding permit (Class A) Special Knowledge Required: Rights of PLWHAS and Basic facts on HIV/AIDS Care
