POSITION SUMMARY

The Medical Case Manager provides comprehensive case management services to pregnant women and new mothers, ensuring access to quality maternal health care services. This role involves assessing clients' medical needs, developing care plans and coordinating services to promote healthy outcomes. Compliance with national medical ethics, laws and reporting requirements is core to this role. In addition, the Medical Case Study manager is actively involved in delivering holistic care to refugee clients, particularly all clients and youth in crisis, by contributing to case management, reintegration, and support services. The Medical Case Manager reports directly to the Head of community services.

KEY RESPONSIBILITIES:

1. Community Engagement and maternal Health Oversight

  • Client Assessment and Care Planning: Conduct medical assessments, develop individualized care plans and monitor client progress
  • Coordinate medical services, appointments and referrals, ensuring clients receive timely and quality care
  • Health Education and counseling: provide health education, counseling and support on maternal health issues, promoting healthy behaviors and self-care
  • Establish and maintain linkages with healthcare providers ensuring clients access necessary services
  • Champion Amani Sasa’s Maternal Health Policy by training staff and promoting safe practices
  • Monitor, review, and update Maternal Health Policy in alignment with local laws and global standards.
  • Attend government-organized and partner trainings or forums related maternal Health.
  • Follow up with participants and families before and after program graduation.

2. Support Operations of the Clients' Program

  • Attend weekly client Program meetings and track client progress.
  • Participate in the selection of clients to join the program.
  • Coordinate with counselors to schedule client sessions.

3. Case Management and Client Support

  • Conduct client assessments, including home visits.
  • Accompany clients to legal, counseling, and medical appointments.
  • Help secure social support services such as emergency shelter or food.
  • Document case notes, follow-ups, and service referrals in coordination with social work team.
  • Maintain accurate and confidential records of maternal health cases and incidents for reporting purposes.

5. Additional Responsibilities

  • Participate in Amami Sasa staff continuing education.
  • Perform any other duties assigned by the COD/CEO.

QUALIFICATIONS:

  • Diploma or Bachelor's degree Nursing, Midwifery or related field
  • Relevant certification in case management or maternal health
  • Minimum 2 years of experience in child protection or related field
  • Strong communication and interpersonal skills
  • Experience with medical records and data management systems
  • Ability to work with vulnerable children and families
